How to Complete a Questionnaire

Questionnaire completion access is required.
Questionnaires are a great way to work through protocol-driven audits, with pre-defined questions being investigated and answered during the audit.
The Questionnaire tab allows an auditor to record the answers to the questions and also report any findings, note observations, create action items or upload evidence.
This article outlines how to complete a questionnaire and is split into the following sections:
1. Questionnaire Responses
Questionnaire responses can be classed as one of three types:
Response Type | Description |
---|---|
Preferred | Indicates there is no issue found for the question and no finding will be created. This is highlighted in green. |
Non-Preferred | Indicates a deficiency was identified during the audit. A finding and at least one action item will need to be created. This is highlighted in red. |
Ignore |
Indicates the question does not apply to the location being audited. |
Note: If your questionnaire has been configured to use scoring then any ignore responses will not be included in the scores.
In addition, icons will identify the status of each stage of the questionnaire response. Details of the icons are outlined in the table below:

2. How to Complete a Questionnaire
- Click Modules, and then Audit Management.
- Click Manage Audits.
- Search for and open the audit you need to update.
- Expand Audit Setup.
- Click Questionnaire.
- Select a Section.
- Click the response dropdown to select your question response.
- Repeat this process for all questions on the questionnaire.
3. How to Report a Finding
If required you can report a finding (such as a non-conformance.
When you select a non-preferred answer the findings button will highlight red.
To report a finding:
- Click on the red Finding button.
- Enter the Finding details.
- If required, set an action item and upload any evidence as an attachment.
