How to Investigate an Incident

Incident edit access is required.
When an incident report is raised it should be investigated.
This article outlines how to perform your investigation and is split into the following sections:
1. Investigation Stages
The table below outlines the steps available to investigate the incident:
Stages | Description |
---|---|
Investigation Questions | A series of questions that should be asked in order to understand the scope of the incident as well as the root cause. |
Contributing Factors |
The various factors that resulted in the incident taking place. In some cases there may only be one contributing factor but in other scenarios there may be several factors that collectively lead to the incident. |
5 Why Methodology | A problem-solving technique that involves asking "why" five times to get to the root cause of a problem. It is commonly used in quality control and management to identify the underlying causes of issues. |
Root Cause Statement | A description defining the root cause. |
Lessons Learned Statement | A description of the insights and recommendations gained from the incident. This may include preventive actions. |
2. How to Investigate an Incident
Important Note: The Detail Report must be completed before you can complete the Investigation Report.
- Click Modules and then Incident Management.
- Click Manage Incident Reports.
- Click on the ID of the incident to begin the investigation.
- Browse to the Investigate section.
- Click on the Incomplete link on the section you need to complete.
- Fill in all required fields.
- Click Save.
- Repeat the process for all required sections.
Note: You can click on the Investigate icon to view a summary of each section.
