A key part of reporting an incident is to capture witness statements.
A witness statement is a written or recorded account of an incident or event by an individual who observed it firsthand. Witness statements can be crucial in investigating incidents and determining the cause and contributing factors. They can provide important details that may have been overlooked or not captured by other means, such as photos or video footage. Witness statements should be factual and objective, and include the date, time, location, and any other relevant details about the incident or event.

1. How to Report a New Incident
- Click Modules and then Incident Management.
- Click Report New Incident.
- Enter a clear an concise title.
- Select the Incident Type.
- Click Create.
- Locate the Witness Information section.
- Set "Are there any Witnesses identified?" to Yes.
- Complete the required information.
- Repeat the steps again for each additional witness.
