The Chain of Custody (COC) is a crucial record that tracks the handling, transfer, and control of samples collected from a workplace during the entire process, from collection to analysis. It ensures the integrity and traceability of the samples, providing a documented trail of custody for legal and quality assurance purposes.

Important Note:

• You must have permissions associated with the Industrial Hygiene role.
• You cannot create a Chain of Custody at a node level.

1. How to Generate a Blank Chain of Custody Form

1. Click Modules and then Industrial Hygiene.
2. Click Chain of Custody.
3. Click Blank COC Form.

4. Select the lab name.
5. Click Run Report.

Important Note: Once created the Chain of Custody (COC) record will allow you to click Submit to Lab. This will create an electronic record that contains both the PDF and an Excel for the lab to fill the results upon analyzing them. The user will get a confirmation window and an email will be generated with the electronic (COC).
